Home adaptation program (HAP)

If you have limitations in carrying out your daily activities, or have difficulty accessing your home or certain essential rooms due to environmental obstacles, the Home Adaptation Program is for you. The occupational therapy assessment will determine your needs and make recommendations in terms of adaptation.

Whether you are an elderly person with a loss of autonomy, whether you have a neurological problem or health effects of a cerebrovascular accident or physical limitations following an accident, you may have certain difficulties in carrying out your daily activities. Challenges could be experienced during your hygiene care in the bathtub or shower, for your transfers to bed or during the preparation of your meals. A risk of falling could also be present. The occupational therapy assessment allows to target your difficulties and highlight the means that would promote your autonomy and your safety in the activities that are important to you.

You do not drive or have you stopped driving due to a significant and persistent impairment? Are you unable to travel by public transport due to mobility limitations? Paratransit is a door-to-door public transportation service offered to people living with limitations that have a significant and persistent impact on their mobility. An occupational therapy assessment will validate your eligibility and complete the application. Note that other professionals can also complete the assessment, depending on the nature of your disabilities.

Assessment for a four-wheeled scooter request

Do you have difficulty moving medium/long distances due to heart or respiratory disease or due to a physical inability to walk for your normal daily activities?
A four-wheeled scooter could possibly meet your needs. The occupational therapy assessment will determine if it is indeed the best equipment for you and validate your eligibility according to the program criteria. Assessment for a parking sticker request for people with disabilities or reduced mobility

Do you have trouble moving safely after parking too far from a store entrance? You cannot leave your loved one alone at the entrance to park afterwards out of fear for their safety?
The disabled parking sticker could meet your needs (if you meet the criteria governed by the SAAQ). As an occupational therapist, I could assess you to determine your eligibility for a parking sticker. Other professionals are also authorized to do so. Assessment of the functional abilities of people with cognitive difficulties

Do you or your loved one suffer from cognitive difficulties that have an impact on the performance of your activities, but especially on your safety at home? Are you concerned because a risk of fire or financial management difficulties are observed? Occupational therapy assessment provides an understanding of the impact of cognitive difficulties on functional abilities, but also, if relevant, to assess cognitive functions by passing certain standardized tests.

Occupational therapy assessment helps clarify the cause of wound development and determine interventions to reduce risk and promote skin recovery (recommendation of therapeutic surfaces, review of positioning, etc.)

When a person displays disruptive behaviour (e.g. agitation or wandering) or a significant risk of falling, their loved ones can sometimes put in place measures to protect them (e.g. a table in front of the chair to prevent them from getting up, bed rails, etc.) This is what we refer to as a control measure or restraint which refers to "equipment or devices that are attached or adjacent to a person's body and restrict the movement of this person. They are usually installed and removed by a third party. They cannot be removed easily by the user and sometimes impose a given position. (CIUSSS MCQ, 2005)
The ministerial orientations maintain that the use of such a measure is minimal and exceptional and must take into account the physical and mental state of the person. An occupational therapy assessment will support you in the search for alternative measures to restraints (e.g.: emergency alert system, use of diversion, revision of the occupational schedule, etc.)

Note that many insurance companies reimburse the costs related to professional services. Contact your insurer to find out. Also, the cost of occupational therapy services is deductible for tax purposes.
